Health Care Power of Attorney

While never a pleasant topic to consider, a devastating accident or crippling illness can leave a person incapacitated and unable to speak for him or herself. In these situations a Health Care Power of Attorney can allow a person to make known in advance what sort of care he or she wants to receive in the event of incapacity.

A Health Care Power of Attorney enables a person (the "Principal") to designate an individual (the "Agent") to make health-care decisions in the event that the Principal lacks capacity and to provide the Agent with guidance as to what sort of treatment the Principal wants.

This important document provides guidance to loved ones in a difficult situation and gives the Principal peace of mind from knowing that his or her wishes will be respected. The following link opens a fillable-forms Health Care Power of Attorney form. This form is designed for Illinois residents who do not already have an existing power of attorney. It is strongly recommended that anyone using this form speak with the person or persons that would be designated as "Agent" in the document and make sure that person is comfortable acting in such a role.
